Important Update for Patients with UHC Medicare HMO Plans
We want to make you aware of an important requirement that may affect your upcoming appointments.
UnitedHealthcare (UHC) Medicare HMO plans require a referral from your Primary Care Physician (PCP) before you can receive care from our providers. The referral must be submitted through UHC and approved before your scheduled visit.
If you have a UHC Medicare HMO plan, please contact your PCP and request a referral for your appointments with our providers. Once the referral has been processed and approved by UHC, we will be happy to schedule your appointment(s).
